Advance Care Planning Service: Marie Curie

The Marie Curie Advance Care Planning (ACP) service offers patients the opportunity to plan for their future care and medical treatment. 

Marie Curie registered nurses will take time to discuss and record the patient's wishes and their decisions regarding future care on the Universal Care Plan (UCP), while they still have the mental capacity to make these decisions. The discussion and creation of the UCP can take place with patients, their family and/or loved ones.

The aims and benefits of the service include:

  • Releasing clinical time through reducing demand on GPs and other healthcare professionals who would otherwise be asked to initiate and complete ACPs for their population
  • Ensuring better uptake of the UCP as a tool for ensuring more people who need it have an ACP
  • Improved outcomes for patients and families (e.g. ensuring more people are able to die in their chosen place of death).

Issues of cultural competence are at the heart of successfully creating ACPs, and Marie Curie nurses are adept at providing a culturally sensitive approach to increase the uptake of ACPs among the global majority population of NCL. 

Marie Curie registered nurses will contact the patient to arrange either a virtual or face-to-face appointment.

Appointments can take place either over the phone, via Microsoft Teams, Zoom or Skype, or in your usual place of residence.


Eligibility Criteria

Inclusions

  • resident of NCL
  • anyone you feel would benefit from a Universal Care Plan
  • anyone who has expressed interest in recording their future wishes.

Marie Curie ACP Team

Any health or care professional can refer a patient by obtaining their consent and then calling the number above with the patient's name and NHS number.
