District Nurse & Planned Care Housebound Service: CLCH

Barnet

Central London Community Healthcare (CLCH) offers a range of community planned care services for housebound Barnet residents.

Community nursing

The community nursing service offers:

  • medication administration
  • wound care, including pressure ulcers
  • suture/clip removals
  • continence assessments
  • bowel care
  • catheter care
  • pressure area care
  • equipment provision (with assisted nursing intervention).

Intermediate care therapy services

The intermediate care therapy service offers:

  • physiotherapy rehabilitation and support for mobility or with managing activities of daily living
  • occupational therapy rehabilitation and support for mobility or with managing activities of daily living.

Falls

The falls service offers:

  • risk assessments
  • activity classes to promote strength/balance
  • education sessions.

Domiciliary phlebotomy

The domiciliary phlebotomy service offers a blood testing service for housebound patients.


Eligibility Criteria

Inclusions

  • housebound residents of Barnet aged 18+.

Complete the Single point of Access Integrated Adult Community Services CLCH - Barnet referral form and send to clcht.plannedcarebarnet@nhs.net

For tissue viability referrals, complete the Barnet Community Tissue Viability & Wound Clinic Referral - CLCH referral form and send to email address above.

For catheter referrals, complete the Urinary Catheter Referral Form – NCL and send to and send to email address above.
