The new Integrated Health and Care Front Door Service in Islington is operational as of the Tuesday 1 October 2024. 

This is a new health and care model that was developed jointly by Whittington Health and Islington's Adult Social Care Services and is based at 222 Upper Street. 

The service is designed to act as a single point of access for all adult social care, urgent community health and hospital discharge referrals for Islington residents.

By creating this single point of access, the service hope to achieve a timelier response to residents' care needs with care plans developed and delivered in a more holistic, multi-disciplinary and coordinated way.

The Integrated Health and Care Front Door Service want to enable residents to live healthy independent lives for as long as possible in their own homes, or the place they call home, and this new model help us towards achieving this ambition. 

How does this affect you?

As a GP or a practice nurse the only notable change for you will be the new integrated referral form available on EMIS which combines the existing community health referral forms for District Nursing and the joint form for Community Rehab/REACH Intermediate Care. 

The referral form is called Islington Integrated Care Referral Form – urgent community health and it and should be emailed to rapidaccess@islington.gov.uk 

All other referral routes for the above-mentioned services will no longer be operational from the 1 October.

  • ambulatory catheter clinics
  • assessment and ordering of equipment to enable care in the home
  • bladder and continence assessments
  • blood tests
  • bowel care
  • catheter care, including re-catherisation
  • chemotherapy management
  • complex wound assessments and management
  • continuing care assessments
  • controlled drug administration
  • end-of-life care
  • healthy living advice
  • intravenous antibiotics
  • leg ulcer clinics
  • long-term condition management
  • lymphoedema management
  • medication management
  • mental capacity assessments
  • nutritional advice
  • PEG tube care
  • pleurex drainage
  • pressure area care
  • pressure ulcer management (category [grade] 2 or lower, for category [grade] 3 refer to the Tissue Viability Service)
  • self-management support
  • support for carers and family
  • symptom control
  • vacuum pump dressings
  • vitamin B12 injections
  • wound care.

Eligibility Criteria

Inclusions

  • housebound Islington residents aged 16+ (unable to receive care outside of their place of residence)
  • district nursing also provides support to patients and their carers to take responsibility for managing their own care, which helps to reduce unplanned admissions to hospital.

Exclusions

  • any resident able to travel to receive care.

District Nursing Message Taking Service

The Message Taking Service is available 24 hours a day.

Referral methods: Email

For District Nursing complete the Islington Integrated Care Referral Form – urgent community health and send to rapidaccess@islington.gov.uk

For referrals to the District Nursing Phlebotomist complete the District Nursing phlebotomist Referral Form - Whittington and send to rapidaccess@islington.gov.uk

Referrals for district nursing should be sent at least 48 hours before the patient’s first visit is required. If your patient needs to be seen sooner, call the Message Taking Service.

Referrals for the administration of medication should be clear and should include the name of the drug, the dose, the dosing time and the length of treatment. The medication record needs to be signed and dated by the prescribing doctor.
