The aims of this End of Life Care (EoLC) service are:

• To ensure that patients are treated in the most appropriate environment, according to their end-of-life wishes, where practical.
• To reduce, across Camden as a whole, the number of avoidable hospital admissions for patients nearing the end of life and thus reducing secondary care spend.
