EoLC

The aims of this End of Life Care (EoLC) service are:

  • To ensure that patients are treated in the most appropriate environment, according to their end-of-life wishes, where practical.
  • To reduce, across Camden as a whole, the number of avoidable hospital admissions for patients nearing the end of life and thus reducing secondary care spend.

Please note that all Action Plans and ADA Audits for EoLC, must come to the camdenlcs@nhs.net inbox.
