The aims and objectives of the care homes locally commissioned service (LCS) are to:

• improve patient satisfaction
• improve the quality of medical support to residents
• improve end-of-life care
• reduce inappropriate admissions to secondary/acute care
• improve medicines management in terms of both quality and cost
• provide a responsive and equitable service which adapts over time to resident feedback and to changes in evidence concerning medical care for care home residents
• provide clinical leadership to improve the overall quality of care for residents in care homes
• deliver appropriate chronic disease management to this population
• encourage the effective use of agreed local care pathways and local health economy resources to reduce inappropriate admissions to secondary care
• improve professional relationships between the provider and care home staff.
