Palliative Specialist Community Care: CNWL

Specialist palliative and end-of-life care is offered in the community to patients with life-limiting illnesses, alongside support for their families and carers.

- For patients in Islington and South Camden
How to Refer
Triage clinician
Call the triage clinician for clinical advice or when a referral is not required (Monday to Sunday, 9am-5pm).
UCLH switchboard
Call the University College London Hospitals (UCLH) switchboard and ask for the air-call palliative care team (Monday to Sunday, 5-8pm).

Complete the All Age Pan-London Specialist Palliative Care Referral form and send to palliative.southcamden@nhs.net
