The Barnet Community Ageing Well’s (CAW) Multi-Disciplinary Team’s (MDT) (formerly Frailty Multi-disciplinary Team) Locally Commissioned Service (LCS) supports GP practices and primary care networks in identifying patient needs, referring to, and participating in, Barnet CAW's MDT meetings.

This enables collaborative case discussions with acute and community healthcare professionals to enhance the coordinated care of the ageing population in Barnet.
