District Nursing (CNWL Senior Nurse)

Please contact the Clinician of the Day (COD) if you have a query about any of your patients, for example, to discuss:
- any unforeseen complexities
- rehabilitation goals and expectations
- episodes of deterioration
- medication
- urgent catheter change
- wound management
- end-of-life care
- long-term condition management
- attendance at a discharge planning meeting
The COD numbers are manned by senior district nursing clinicians via a direct mobile phone number and an email inbox. There is also a triage therapy dead on duty.
Eligibility Criteria
Inclusions
- Must be currently on the caseload for the district nursing team
Exclusions
- patients who are not under the care of the district nursing service
How to Refer
Clinician of The Day (COD) day teams are available 7 days a week 8.30am-5pm
