- any unforeseen complexities
- rehabilitation goals and expectations
- episodes of deterioration
- medication
- urgent catheter change (for referral criteria, please see the Urology page)
- wound management
- end-of-life care
- long term condition management
- attendance at a discharge planning meeting
Eligibility Criteria
Inclusions
Must be currently on the caseload for the District Nursing Team
Exclusions
- patients who are not under the care of the district nursing service

Twilight/Overnight
Response time: 15 min (maximum)
Availability: 24/7
