• any unforeseen complexities
• rehabilitation goals and expectations
• episodes of deterioration
• medication
• urgent catheter change (for referral criteria, please see the Urology page)
• wound management
• end-of-life care
• long term condition management
• attendance at a discharge planning meeting

Inclusions

Must be currently on the caseload for the District Nursing Team

Exclusions

• patients who are not under the care of the district nursing service
