Please contact the Clinician of the Day (COD) if you have a query about any of your patients, for example, to discuss:

• any unforeseen complexities
• rehabilitation goals and expectations
• episodes of deterioration
• medication
• urgent catheter change
• wound management
• end-of-life care
• long-term condition management
• attendance at a discharge planning meeting

The COD numbers are manned by senior district nursing clinicians via a direct mobile phone number and an email inbox. There is also a triage therapy dead on duty.

Eligibility Criteria

Inclusions

• Must be currently on the caseload for the district nursing team

Exclusions

• patients who are not under the care of the district nursing service
