Expansion of the Universal Care Plan (UCP) into a personalised care and support plan is planned for December 2024.

To help users and front-line clinical staff prepare, NCL ICB will be hosting webinars to discuss the upcoming changes, explain their impact on current users, and provide an opportunity to address any questions (see event listings below for details).

Additional resources will be released in the coming weeks, and key messages are regularly updated on the London-wide UCP website. 

In-progress form deadline

Once the UCP has been expanded, all submitted forms will be migrated to the new template. There is a risk that some in-progress forms will not be migrated, therefore please complete these before the December go-live date.

Please note: in-progress forms cannot be viewed by urgent care services.

Access to the editable version of the UCP from the London Care Record within TPP SystmOne and EMIS is also available.
