The Barnet Social Prescribing Link Worker Service supports patients whose health and wellbeing are affected by non-medical challenges that are usually social, practical, or financial.

Social Prescribing Link Workers take time to listen and understand what matters most to you. They work with patients holistically to explore their circumstances, strengths, and goals. Together, they will create a personalised action plan to help patients manage their situation and improve wellbeing. This may include information, guidance, and connecting you to local services and community support.

The service can help with a wide range of non-medical issues, including:

  • Addiction and substance misuse support
  • Mild-moderate anxiety, depression, and emotional wellbeing
  • Benefits and welfare advice
  • Bereavement and cancer support
  • Carer and parental support
  • Counselling and talking therapies
  • Debt and financial advice
  • Dementia and learning disability support
  • Diet, nutrition, and physical activity
  • Disability and mobility support
  • Domestic abuse support
  • Education, employment, and training
  • Falls prevention
  • Food support
  • Housing advice
  • Loneliness and social isolation
  • Refugee and asylum seeker advice

 

 


Eligibility Criteria

Inclusions

  • adults (18+) who have a non-medical issue that is affecting their health and wellbeing
  • registered with a Barnet GP practice

Exclusions

  • severe and unstable mental health disorders.

How to Refer

EMIS form

Referral methods: EMIS managed referral

Referrals are generated through EMIS via the elemental SPx route, as this referral is then generated into the new referrals listed on elemental, which is the social prescribing case management platform. Method

  1. Find the patient you wish to refer on EMIS and click on 'my record'.
  2. Click on the 'elemental SPx' button on the left-hand side of the screen.
  3. Click on 'add new express referral' on the right-hand side of the screen.
  4. Confirm patient details are correct, as these auto-populate from EMIS.
  5. Add your PCN Hub under 'refer to centre' (this then repopulates the establishment/surgery).
  6. Continue down the referral and add the reason (which auto-populates), or click on 'other' to free text.
  7. Add any additional information needed to support the referral, such as medication needs or risks associated with the patient, or additional information such as whether the patient is hard of hearing, needs a translator, or whether you need to speak with the patients' carer etc.
  8. Click the box to state the patient consented.
  9. Click 'generate the referral' once ready to submit.

Self-referral

The Barnet Social Prescribing Service operates a self-referral route in 7 out of 8 of the PCN’s.

All PCN’s apart from PCN5 have a self-referral link.

Send an email and the GP surgery of the patient for the correct self-referral link.
