Move4You: NCL Cancer Physical Activity Rehabilitation Service
NCL WideProvided By
The Move4You programme is a 12-session guided physical activity programme providing ongoing support for 12 months, tailored to individual needs and delivered by specialist activity instructors, offering both face-to-face and virtual sessions to help individuals engage in a variety of physical activities before, during, and after cancer treatment.
An EMIS search has been created to support you to identify patients who are eligible for this service in your GP practice.
Eligibility Criteria
Inclusions
- aged 18+ with a cancer diagnosis in the last five years
- lives in or has access to healthcare in Barnet, Camden, Enfield, Haringey or Islington.
How to Refer
Move4You Online Portal
Referrals can be made using the online portal. The form is submitted to the Move4You team, who will invite the patient to attend an initial assessment.
