From Monday 9 September 2024, all community deaths will need to be referred to the local Medical Examiner before the death can be registered.

All Medical Certificate of Cause of Death (MCCD) will require a medical examiner's assurance to be legally valid.

Inclusions

  • if a patient on your list dies outside of hospital.

Exclusions

  • if a patient on your list dies in hospital.

How to Refer

EMIS form

Referral methods: Email

Please complete the referral form before you log the patient as deceased on EMIS.

For Camden referrals, complete the Medical Examiner Referral – UCLH form and send to uclh.camdenme@nhs.net

For Barnet referrals, where patients live across the borough border, complete the Medical Examiner Community Death Referral - NWL New form and send to rf-tr.bhmeo@nhs.net

For all other referrals complete the Medical Examiner Referral Form (Not for UCLH) - Community Deaths - NCL and send to the relevant team.
