All community deaths will need to be referred to the local Medical Examiner before the death can be registered.

All Medical Certificate of Cause of Death (MCCD) will require a medical examiner's assurance to be legally valid.

How to Refer

EMIS form

Referral methods: Email

Complete the referral form before you log the patient as deceased on EMIS.

  • NCL: complete the Medical Examiner Service Referral – RFH form send to rf-tr.rfmeo@nhs.net
  • Barnet: where patients live across the borough border, complete the Medical Examiner NCL Community Referral (not for UCLH) form and send to rf-tr.rfmeo@nhs.net
  • Camden: complete the Medical Examiner Camden Referral – UCLH form and send to uclh.medicalexaminer@nhs.net
  • Enfield/Haringey: complete the Medical Examiner NCL Community Referral (not for UCLH) form and send to northmid.medicalexaminer@nhs.net
  • Islington: complete the Medical Examiner NCL Community Referral (not for UCLH) form and send to hh-tr.medicalexaminer@nhs.net
