Medical Examiner Service
NCL WideProvided By
Referral to a local Medical Examiner is necessary for all community deaths prior to registration, ensuring the Medical Certificate of Cause of Death (MCCD) is legally validated.
Eligibility Criteria
Inclusions
- if a patient on your list dies outside of hospital.
Exclusions
- if a patient on your list dies in hospital.
How to Refer
EMIS form
Referral methods: Email
Complete the referral form before you log the patient as deceased on EMIS.
- Barnet: complete the Medical Examiner Service Referral – RFH form and send to rf-tr.rfmeo@nhs.net
- Camden: complete the Medical Examiner Camden Referral – UCLH form and send to uclh.medicalexaminer@nhs.net
- Enfield/Haringey: complete the Medical Examiner Enfield & Haringey Referral – NMH form and send to rf-tr.northmid.medicalexaminer@nhs.net
- Islington: complete the Medical Examiner Islington Referral – Whittington form and send to hh-tr.medicalexaminer@nhs.net
