Eligibility Criteria

Inclusions

  •  Aged 18 years or over
  • Who lives in the borough of Haringey
  • Who has been bereaved of someone following a terminal illness
  • Waiting period of three months from point of bereavement before a referral can be accepted (either self-referrals or from other referrer)
  • NLH exempted from this
  • Start date 1 April 2022

Referrals can be made directly to the First Contact team at St Joseph's Hospice by telephone or email (using the referral form).
