Appointments must be booked in advance
The adult phlebotomy services at Finchley Memorial Hospital Community Diagnostic Centre are offered by Central London Community Healthcare (CLCH).
Booking an appointment
Patients can access all phlebotomy appointments both online and by phone (routine, urgent, INR and GTT appointments). This information is provided to the patients on the blood form.
Patients should arrive no more than 10 minutes before the scheduled time, bringing their confirmation email or booking reference number.

- aged under 12 years
- specialist blood tests
- sample drop-off point.
How to Refer
There is no facility to print a blood test form at the CDC, so please send patient with a printed paper blood form.
Patients should book an appointment online via Swiftqueue.
If patients do not have internet access, they can book via phone.t: 020 8349 7460 (open 8am-11am).
