The General Practice Change Management Support provides bespoke, hands-on support to help GP practices improve patient access, care for people with long-term conditions and digital transformation. Support is delivered through a local borough team.
What the offer can support
- Access: helping practices understand demand, capacity, and explore practical improvements that fit a team's needs
- Digital: supporting the effective use of digital tools and workflows
- Long-term condition care: supporting the sustainable delivery of the Model of Care, embedding personalised care and meeting targets.
How it works
The approach is led by local clinical and non-clinical facilitators, using quality-improvement methods and peer learning tailored to individual practices. Practices are supported to find ways of working that reduce pressure and free up time in the long term.
Initially, support will focus on practices that need the most help in these areas, alongside a universal offer available to all practices. This is a supportive offer and not performance management or assurance activity.
Meet the team
The programme is led by a team from Haringey and Islington GP Federations and the Qualitas Consortium, with all north cental London GP Federations providing local support and delivery.
