The Community Specialist Palliative Care team in south Camden, provided by Central and North West London NHS Foundation Trust (CNWL), provides specialist palliative and end-of-life care in the community to patients with life-limiting illness.
The service also offers support to families and carers.
How to Refer
Triage clinician
Call the triage clinician for clinical advice or when a referral is not required (Monday to Sunday, 9am-5pm).
UCLH switchboard
Call the University College London Hospitals (UCLH) switchboard and ask for the air-call palliative care team (Monday to Sunday, 5-8pm).
