Children's Community Nursing Team: NMUH

The NMUH Children's Community Nursing Team (CCNT) consists of qualified children's nurses and support workers. They are dedicated and committed to delivering care closer to home for children and young people within the boroughs of Enfield and Haringey.
The team works closely with other health professionals and organisations including children's wards, GPs, social services, health visitors, school nurses and child development teams.
Any health professional can refer to the NMUH Children's Community Nursing Team.
The CCNT visits children at home with:
- oncology (cancer) conditions
- chemotherapy administration
- wounds (dressings after surgery, burns/scalds)
- removal of sutures and clips
- administration of intravenous antibiotics
- injections
- colostomy care
- home oxygen
- nasogastric care and education
- tracheotomy care
- palliative care
- cardiac conditions
- discharge planning and coordination of equipment
- gastrostomy care
- endocrine disorders
Referrals are accepted from:
- accident & emergency departments
- general practitioners (GPs)
- hospital and community paediatricians
- therapists
- tertiary hospitals
- health visitors
- practice nurses
Eligibility Criteria
Inclusions
- The team will see any child or young adult aged <16 with a nursing need.
How to Refer
EMIS form
Referral methods: e-Referrals
Referrals to the CCNT are separated by postcode. The service accepts referrals from any borough as long as the child has a nursing need and is living within the catchment area.
Complete the Community Children's Nursing Referral NCL form for Haringey and Enfield residents (post code beginning with 'N') and send to:

For Enfield residents (post code beginning with 'E'), send the referral form to:
e: rf-tr.ChildrensHomecareTeam@nhs.net
Referrers should add as much information as applicable, such as discharge summaries, clinic letters, feed plan, drug charts, PICC line details, follow-up arrangements and relevant contact details.
