Children's Community Nursing Team: WH

Whittington Health Community Children's Nursing (CCN) are registered children's nurses who provide nursing care and support to children at home or in school, coordinating with local paediatricians, GPs, and specialists for children with long-term or complex needs. The Paediatric Primary Care team provide nurse-led services for conditions like eczema and asthma, and neonatal nurses collaborate with hospitals for care and advice in Islington.
Eligibility Criteria
Inclusions
- children aged 0-18 who have an Islington GP and a nursing need. This includes, but is not limited to, children who require:
- enteral feeding support
- home oxygen administration
- wound care
- intravenous medication
- injectable medication
- artificial ventilation
- palliative care
- oncology care
- central venous access device care
Exclusions
- the team are unable to assist with administration of oral medication

Complete the Community Children’s Nursing Referral NCL form and send to whh-tr.IslingtonChildrensNursing@nhs.net
Referrers should add as much information as applicable, such as discharge summaries, clinic letters, feed plan, drug charts, PICC line details, follow-up arrangements and relevant contact details.
