  • multi-disciplinary team working
  • named Care Co-ordinator for all case management
  • clinically integrated MSK services

The Camden MSK website (see external links below) has online resources for pain management support, home exercise, and emotional well-being.

How to refer

EMIS form

Referral methods: e-Referrals

With the exception of self-referrals into physiotherapy (see below), only e-Referrals are accepted. Find and complete the Camden MSK Referral Form (v.1.5) from the EMIS Forms page (Word version available in Downloads), and send via the e-Referrals system using the following steps:

  1. Specialty: Physiotherapy
  2. Clinic type: Musculoskeletal
  3. Clinic name: Camden MSK
  4. Send for triage

Once referral has been received, it will be triaged and directed to the most appropriate service in Camden MSK (please see Downloads for a guide).

Urgent rheumatology referrals

For urgent rheumatology referrals, tick the Urgent box on the referral form. Complete form with as much referral information as possible. Urgent referrals are sent directly to Rheumatology on day of receipt and followed up with a confirmation of receipt by the Camden MSK admin team.

Please note that all patients referred to spinal neurosurgery will be managed by Camden MSKs Single Point of Referral for triage.

Self-referral Patient Leaflet

Self-referral into Physiotherapy

Self-referral by telephone is the quickest way to access the service. Please provide patient leaflet (see Downloads).
