Palliative Care: St John's Hospice
CamdenProvided By
St John's Hospice can provide specialist advice for symptom control issues, as well as emotional, social and spiritual support, working alongside other community and acute health and social care professionals involved in the patient's care.
Eligibility Criteria
Inclusions
- aged 18+ and living in Camden/registered with an Camden GP
- diagnosis of potentially life-limiting illnesses, with unresolved complex needs that cannot be met by the referring team (these needs may be physical, psychological, social and/or spiritual)
- advanced progressive life-limiting illnesses, likely to be in the last year of life
