Neurological and Stroke Service: CNWL

Camden Neurological and Stroke Service (CNSS) is made up of a team of clinical psychologists, occupational therapists, physiotherapists, speech therapists and a neuro-navigator.
The service provides specialist rehabilitation in community settings, including a range of assessments and treatments in groups and for individuals.
Eligibility Criteria
Inclusions
CNSS treats active or focal neurological impairments that would benefit from a specialist neurological approach to rehabilitation, including:
- stroke
- Parkinson's disease
- multiple sclerosis
- traumatic brain injury
- anoxic brain injury
- spinal cord injuries
- neuro-muscular disease
- motor neurone disease.
Exclusions
- where the main issue impacting on function is not neurological, the patient should be referred to the integrated primary care team for specialist generic rehabilitation (e.g. a patient with an old stroke who has been self-managing but has now broken a hip)
- patients with equipment or social care needs only should be referred to Adult Social Care
- patients with musculoskeletal injuries should be referred to Camden MSK.

Complete the Camden Integrated Adult Community Services - CNWL referral form and send to the Camden Central Access Team: camdenreferrals.cnwl@nhs.net
When referring, please give comprehensive information about the patient's:
- diagnosis
- impairment (be as specific as possible, including any recent change in impairment)
- function (be as specific as possible, including any recent change in function)
- aim of referral/treatment
- care needs.
The referral will be screened to ensure that the patient is seen by the appropriate member(s) of the team.
