The anticoagulation service at UCLH is a nurse-led clinic supported by a multi-disciplinary team.

The clinic initiates and monitors patients on vitamin K antagonists (such as warfarin) and direct oral anticoagulants (DOAC).

Patients who have been initiated on warfarin should have been provided with an NPSA oral anticoagulant therapy record book (see Downloads) by the hospital clinic. This booklet should be taken with them to any hospital/GP appointments and when collecting anticoagulant prescriptions.

Adults

  • in need of anticoagulation
  • already on anticoagulation that requires monitoring

Exclusions

  • Children
