Anticoagulation Service: WH

NCL Wide

The Whittington Hospital Anticoagulation Service is a pharmacist-led service; the team is supported by a Haematologist and Cardiologist.

The services include:

  • oral anticoagulation monitoring clinic
  • initiation and monitoring of patients on vitamin K antagonists (such as Warfarin) 
  • support for patients who wish to self-test 
  • initiation of direct oral anticoagulants (DOAC).

Patients who have been initiated on warfarin should have been provided with a National Patient Safety Agency (NPSA) oral anticoagulant therapy record book by the hospital clinic.

This booklet should be taken to any hospital/GP appointments and when collecting anticoagulant prescriptions.


Eligibility Criteria

Inclusions

Adults

  • requiring oral anticoagulation initiation
  • already on oral anticoagulation and require monitoring 

Exclusions

  • Children (under 18yrs)
