Prostate Stratified Follow Up LCS

The aim of the Primary Care Stable Prostate Cancer Locally Commissioned Service (LCS) is to provide enhanced support for stable prostate cancer patients in primary care. The service includes an annual holistic needs assessment (HNA) and PSA monitoring. This service supports North Central London ICB’s strategic objective of moving care closer to home. It will also ensure primary care is funded for work being moved from secondary to primary care.

Community Lymphoedema services

Lymphoedema is a chronic swelling due to a failure of the lymphatic system. It is a chronic condition and is not curable and unfortunately, clinical sequelae when lymphoedema is not actively managed can expose the patient and, hence commissioners, to deteriorating physical function and the requirement for escalated clinical intervention. 

Lymphoedema can be alleviated by appropriate active management, but without this it will progress and become increasingly difficult to manage. This means that risk reduction and management strategies should cover the patient’s entire lifespan. The incidence and prevalence of lymphoedema is increasing as more patients survive cancer.
