Nutrition Support Service: CLCH
BarnetCLCH's nutrition and dietetic teams assess dietary intake and provide nutrition advice to help people understand the impact of diet on their health and wellbeing, helping people live longer, healthier lives.
The Barnet team assesses patients in their own home, in nursing homes or in outpatient clinic settings. Assessments can also be carried out virtually, by telephone or video call.
The Nutrition Support Service (NSS) offers advice and help to patients who need oral nutrition supplements for malnutrition. The Home Enteral Tube Feeding Service supports patients who are unable to meet their requirements orally. The service also includes dietetic clinics for weight management to help with obesity.
All services run from Monday to Friday, 9am-5pm, unless stated otherwise.
Referrals for home enteral tube feeding will generally come via the acute hospital dietetic service or rehab dietetic service when the patient is discharged with a feeding tube in situ-feeding. CLCH's community team will accept referrals from allied healthcare professionals, GPs and other healthcare professionals.
Referrals can be made to the NSS and the weight management service by GPs, allied healthcare professionals, hospital dietitians and care home staff, providing they meet the appropriate criteria.
Eligibility Criteria
Inclusions
Please note, the NSS follows a strict commissioning criterion and will only accept patients that meet this. These services are only suitable for patients that live in Barnet, are registered with a Barnet GP and are aged 18+.
Home Enteral Tube Feeding
- any patient who has a feeding tube in situ, for either short or long term
- some patients will be under our service for feeds and ancillaries, whilst their dietetic care will be under other team. These patients may have conditions such as neurologic conditions, active cancer treatment or eating disorders.
Nutrition Support
- has a BMI <18.5kg/m² (no diagnosis of eating disorder)
- BMI 18.5-20kg/m² and unplanned weight loss >5% within the last 3-6 months (with evidence)
- unplanned weight loss >10% within the last 3-6 months (with evidence)
- prescribed oral nutritional supplements
- mid-upper arm circumference (MUAC) <23.5cm and unplanned weight loss >5% (with evidence)
Weight Management
White and European population
- BMI 40+
- BMI >35 plus two co-morbidities*
Black, Asian and minority ethnic population
- BMI 37.5+
- BMI >32.5 plus two co-morbidities*
*These may include: pre-diabetes, cardiovascular disease, dyslipidaemia, hypertension, diagnosed sleep apnoea.
Please note, the service is not commissioned to deliver weight loss injections. Please ensure you have had this discussion with the patient before referral. The NSS only offers conservative weight management support (diet and lifestyle).
