CLCH's nutrition and dietetic teams assess dietary intake and provide nutrition advice to help people understand the impact of diet on their health and wellbeing, helping people live longer, healthier lives.

Depending on need, clients are seen in a home visit, nursing home or outpatient clinic setting. Clients can also be assessed virtually, by telephone or video call.

The nutrition support service (NSS) provides advice and support to patient’s requiring oral nutrition support for malnutrition and home enteral tube feeding for patients unable to meet their requirements orally. The service also provides weight management dietetic clinics for obesity.  

All services run from Monday to Friday, 9am-5pm, unless stated otherwise.

Referrals for home enteral tube feeding will generally come via the acute hospital dietetic service when the patient is discharged with a feeding tube in situ-feeding may be short or long-term. CLCH's community team will accept referrals from GPs and allied healthcare professionals to assess if a patient is appropriate for tube placement and initiation of feeding in hospital.

Referrals can be made to the nutrition support malnutrition service by GPs, allied healthcare professionals, hospital dietitians and care home staff, providing they meet the appropriate criteria.

They can also be made to the service's weight management dietetic clinics by GPs and allied healthcare professionals, providing they meet the appropriate criteria.


Eligibility Criteria

Inclusions

Please note, services are only suitable for patients that live in Barnet, are registered with a Barnet GP and are aged 18+ years; patients who do not meet this criterion will not be accepted.

Home Enteral Tube Feeding:

  • Any patient who has a feeding tube in situ
  • Patients with a neurology diagnosis (such as MS and MMD) will need to be referred to Royal Free Hospital.

Nutrition Support:

  • Has a BMI <18.5kg/m² (no diagnosis of eating disorder)
  • BMI 18.5-20kg/m² and unplanned weight loss >5% within the last 3-6 months (with evidence)
  • Unplanned weight loss >10% within the last 3-6 months (with evidence)
  • Prescribed oral nutritional supplements
  • Mid-upper arm circumference (MUAC) <23.5cm and unplanned weight loss >5% (with evidence)

Weight Management:  

White and European population:

  • BMI 40+
  • BMI >35 plus two co-morbidities*   

Black, Asian and minority ethnic population:

  • BMI 37.5+
  • BMI >32.5 plus two co-morbidities*   
    *These may include: pre-diabetes, cardiovascular disease, dyslipidaemia, hypertension, diagnosed sleep apnoea.
