Integrated Health and Care Front Door Service

Islington

The Integrated Health and Care Front Door Service in Islington is a new health and care model that was developed jointly by Whittington Health and Islington's Adult Social Care Services and is based at 222 Upper Street. 

The service is designed to act as a single point of access for all adult social care, urgent community health, hospital discharge referrals and for referrals into four community health services (district nursing, general rehab and falls therapists, stroke/neuro and speech and language therapists and urgent community therapists).

The service aims to provide a timely response to residents' care needs with care plans developed and delivered in a more holistic, multi-disciplinary and coordinated way.

The Integrated Health and Care Front Door Service aims to enable residents to live healthy independent lives for as long as possible in their own homes, or the place they call home, and this new model help us towards achieving this ambition. 

How does this affect you?

This service is for you if you would need to refer to adult social care, safeguarding or to any of the following four community health services:

  • Islington District Nursing
  • Urgent Community Therapists (formerly part of REACH)
  • Localities Therapies General Rehab/Fall (formerly part of REACH)
  • Localities Therapies Stroke/Neuro and SLT (formerly known as CRT or ICRT i.e. Islington Community Neuro-Rehabilitation Team)

Complete the Social Care and Rehabilitation Team Referral – Islington and send to ASCReferrals@islington.gov.uk

For referrals for Islington Adult Social Care Service. This includes a request for a Care Act assessment, an Occupational Therapy Assessment and/or a review of existing care packages.

For referrals for Safeguarding Concern. This is for any concerns you have about a patient’s care. (For urgent support call the Emergency Duty team using the number above).

Referrals for Islington Integrated Health and Care
Complete the Islington Integrated Health Referral Form – District nursing and Community Therapists and send to rapidaccess@islington.gov.uk

The integrated referral form available on EMIS has been collaboratively designed to streamline referrals to the following 4 services:

  • Islington District Nursing service
  • Urgent community therapists (formerly part of REACH)
  • Localities Therapies General Rehab/Fall (formerly part of REACH)
  • Localities Therapies Stroke/Neuro and SLT (formerly known as CRT or ICRT i.e. Islington Community Neuro-Rehabilitation Team)

If the person you are referring has multiple health needs, they can all be consolidated within this form, rather than submitting separate referrals for these community services.

Self-referral

Islington Adult Social Care Request for Service Form
Patients can complete an Adult Social Care online self-referral form if they would like to request any support from Islington Adult Social Care for themselves or on behalf of somebody else. 

Islington Adult Social Care Safeguarding Concern Form
Patient can also complete a Safeguarding online self-referral form to inform of any concerns they have about their own safeguarding or for a local resident or family member.
