Integrated Health and Care Front Door Service
IslingtonThe new Integrated Health and Care Front Door Service in Islington is operational as of the Tuesday 1 October 2024.
This is a new health and care model that was developed jointly by Whittington Health and Islington's Adult Social Care Services and is based at 222 Upper Street.
The service is designed to act as a single point of access for all adult social care, urgent community health and hospital discharge referrals for Islington residents.
By creating this single point of access, the service hope to achieve a timelier response to residents' care needs with care plans developed and delivered in a more holistic, multi-disciplinary and coordinated way.
The Integrated Health and Care Front Door Service want to enable residents to live healthy independent lives for as long as possible in their own homes, or the place they call home, and this new model help us towards achieving this ambition.
How does this affect you?
As a GP or a practice nurse if you would like to refer to any of the following 4 services:
- Islington District Nursing service
- Urgent community therapists (formerly part of REACH)
- Localities Therapies General Rehab/Fall (formerly part of REACH)
- Localities Therapies Stroke/Neuro and SLT (formerly known as CRT or ICRT i.e. Islington Community Neuro-Rehabilitation Team)
Use the referral form called Islington Integrated Health Referral Form – District nursing and Community Therapists and email it to rapidaccess@islington.gov.uk
Contact the Islington Integrated Health and Care Front Door team if you have any specific operational questions on 020 7527 2299.
The processes for referring your patients to adult social care services for assessment or raising a safeguarding issue remain the same using the referral forms available at the bottom of this page.

For emergencies outside of normal office hours, call the Emergency Duty Team.
Islington Adult Social Care Request for Service Form
For requests for a Care Act Assessment, an Occupational Therapy Assessment, or a review of an existing care package.
Islington Adult Social Care Safeguarding Concern
Please use this form for any safeguarding concerns.

Referrals for Islington Integrated Health and Care
The integrated referral form available on EMIS has been collaboratively designed to streamline referrals to the following 4 services:
- Islington District Nursing service
- Urgent community therapists (formerly part of REACH)
- Localities Therapies General Rehab/Fall (formerly part of REACH)
- Localities Therapies Stroke/Neuro and SLT (formerly known as CRT or ICRT i.e. Islington Community Neuro-Rehabilitation Team)
If the person you are referring has multiple health needs, they can all be consolidated within this form, rather than submitting separate referrals for these community services.
