Urgent Community Response Team

Islington

Urgent Community Therapists (formerly part of REACH) provides urgent community therapy to avoid hospital admission or prevent re-admissions; by facilitating discharges (and emergency discharge) where therapy is indicated. 

The team works in collaboration with adults/older people and their relatives/carers to promote health and maximise independence in mobility, working closely with acute, intermediate and primary care services, social and voluntary agencies, to provide seamless health and social rehabilitation for patients.

The service is comprised of occupational therapists, physiotherapists, and rehabilitation assistants and operates: Mon-Fri 9am-5pm.

The Islington Integrated Health and Care Front Door referral pathway allows GPs to refer patients into Adult social care services, District nursing and Community Therapists. Should your patients have multiple needs. See the Integrated Health and Care Front Door Service page for more information

    • with rehab needs/goals
    • willing to participate in rehab in their community
