Localities Therapists for General Rehab and Falls
IslingtonThe Localities Therapies for General Rehab and Falls is a team that works in collaboration with adults/older people and their relatives/carers to promote health and maximise independence in mobility, communication, work, leisure and everyday activities.
The team work closely with the acute, intermediate, primary care services, and social and voluntary agencies, to provide seamless health and social rehabilitation for patients. The team is comprised of occupational therapists, physiotherapists, and rehabilitation assistants.
This service operates Mon-Fri 9am-5pm.
The referral pathway is through the integrated health and care front door. To refer you patients see the How to Refer section below.
The Islington Integrated Health and Care Front Door referral pathway allows GPs to refer patients into Adult social care services as well as to District nursing and Community Therapists, should your patients have multiple needs. - with rehab needs/goals
- willing to participate in rehab in their community

Complete the Islington Integrated Health Referral Form – District nursing and Community Therapists and send to rapidaccess@islington.gov.uk
Referrers should consider the following alternative services before referring to Islington Locality Therapies:
- Musculoskeletal Physiotherapy
- Falls medical clinic (refer via GP)
- Integrated Community Ageing Team
- Occupational Therapy Social Services

Please note: Self-referrals are not accepted unless the patient has already been under the care of the Rehab and Falls team and is within six months of their discharge date. They should use the contact details on their discharge form.
