Localities Therapists for General Rehab and Falls
IslingtonLocalities Therapists for General Rehab and Falls offers rehabilitation by occupational therapists, physiotherapists, and rehabilitation assistants to enhance mobility, communication, work, leisure, and everyday activities, promoting health and independence while collaborating with various health, care, and voluntary agencies for comprehensive patient support.
Eligibility Criteria
Inclusions
- for <65 falls only: evidence of a fall(s) in the last month that has been screened out from urgent criteria/referred from urgent response. For example, at risk of hospital admission, physical deterioration
- patient living alone, or with caring responsibilities
- showing recent/sudden decline in mobility
- with no care needs but rehab goals
- patient requiring required mobility aid or assessment.
Exclusions
- under 65 yrs
- fall due to alcohol or substance misuse, active alcohol or substance misuse
- referral for musculoskeletal impairments causing falls
- underlying neurological conditions
- patient discharged from falls team in last 12 months and no new fall reported since discharge
- manual handling assessment only
- cognitive screening assessment without rehabilitation need
- occupational therapy equipment only can go to social services.

Please note: Self-referrals are not accepted unless the patient has already been under the care of the Rehab and Falls team and is within six months of their discharge date. They should use the contact details on their discharge form.
