COPD Pulmonary Rehabilitation: CNWL

Camden

Face-to-face Pulmonary Rehabilitation runs out of only the Peckwater Centre in Kentish Town. Remote Pulmonary Rehabilitation delivered via Zoom remains an option for patients to complete from their homes. Efforts are ongoing to locate a second face-to-face site to deliver Pulmonary Rehabilitation. 

We are able to arrange patient transport and interpreters for those who require these services.

Refer eligible patients into the service and a member of the team will contact them within two weeks to offer one of the following:

  • Advice and signposting to online/digital resources (e.g. Asthma and Lung UK, COPD YouTube channel) as appropriate
  • Provision of a basic home exercise programme to increase access to the formal PR Program
  • Remote pulmonary rehabilitation
  • Face-to-face pulmonary rehabilitation 

Pulmonary Rehabilitation is a programme of moderate intensity exercise and education for patients with chronic lung disease and impacted by breathlessness. Patients are expected to attend twice per week for six weeks. Pulmonary rehabilitation should be offered to all patients MRC 3 and above, as well as any patients who feel functionally limited by breathlessness, or exacerbate frequently; see NICE COPD guidelines 2018 link in the Resources section.

Please include patient spirometry with tracing confirming diagnosis of COPD on referral and review that the patient meets criteria for referral (see Eligibility Criteria below).


Eligibility Criteria

Inclusions

  • registered with Camden GP
  • COPD diagnosis confirmed with quality assured spirometry
  • patients with a confirmed diagnosis of Interstitial Lung Disease, Bronchiectasis, or Chronic Stable Asthma who have been reviewed by secondary care in the last 12 months
  • willingness to participate
  • MRC score of 3-5
  • MRC score of 1 or 2 with a history of repeated exacerbations or limited by breathlessness
  • sufficient mobility
  • optimal medical management
  • ability to travel for face-to-face pulmonary rehabilitation (transport available if otherwise unable to attend)
  • internet capabilities at home (for remote PR) and if possible a laptop/tablet (a tablet can be arranged if the patient only has internet at home).

Exclusions

  • unstable cardiac disease including, but not limited to: acute LVF; severe aortic stenosis; unstable angina; uncontrolled arrhythmias; suspected or known dissecting aneurysm
  • AAA > 5.5cm
  • uncontrolled diabetes
  • symptomatic postural hypotension
  • post-covid syndrome fatigue that is unmanaged
  • frequent dizzy spells or incidence of loss of consciousness that has not been investigated
  • 3 or more falls in the last year- should be referred to the falls team prior to PR
  • uncontrolled musculoskeletal pain
  • cognitive impairment which severely restricts compliance with the programme.

Refer via the EMIS form if patient meets referral criteria and is able to commit to programme.
