Provided By
The COPD Community Respiratory Service provides spirometry for diagnosis, pulmonary rehabilitation for individuals with COPD and other chronic lung diseases, and supports home oxygen therapy use. It also offers home visits for COPD self-management and respiratory clinics for patients with symptoms that deteriorate beyond primary care management.
Eligibility Criteria
Inclusions
- aged 18+
- registered with a Camden GP
- confirmed diagnosis of COPD (for home pathway management).
Exclusions
Not suitable for those who:
- require full pulmonary lung function testing for diagnosis
- assessment for complex interventions such as type 2 respiratory failure or hypercapnia, assessment for lung volume reduction interventions.

Complete the Camden Integrated Adult Community Services - CNWL referral form (attaching any relevant discharge, outpatient reports or recent spirometry) and email to the Camden Central Access Team: camdenreferrals.cnwl@nhs.net
