Stroke Rehabilitation Community Service

Enfield

The Community Stroke Rehabilitation Service in Enfield consists of physiotherapists, occupational therapists, speech and language therapists and rehabilitation assistants. It works closely with colleagues from the voluntary sector to provide support for life skills, stroke navigation and social integration.

The service:

  • enables medically stable patients to be discharged early from the Hyper Acute Stroke Units
  • provides community stroke rehabilitation in, or close to, the patient's home
  • supports stroke patients to achieve a good quality of life, maximise independence and wellbeing, and reduce mortality and long-term disability.

This service is delivered from a number of locations, including patients' homes, workplaces, clinics and community venues, such as nursing homes and residential care.


Eligibility Criteria

Inclusions

Enfield residents who:

  • are aged 18+
  • are registered with a GP
  • have recently suffered a stroke and are being discharged from inpatient stroke services
  • have had a stroke and require multidisciplinary rehabilitation input.

Self-referral

The service accepts self-referrals from patients and carers for patients who have previously received an episode of care.
