Early Stroke Discharge: CLCH
BarnetProvided By
The Early Stroke Discharge (ESD) service is for Barnet residents who have suffered a stroke and their family/carers. The service aims to make contact with a client within 24 hours of being home.
This service can provide equipment on day of discharge if requested in advance (usually five days).
Eligibility Criteria
Inclusions
- Have a confirmed acute stroke diagnosis (by MRI/CT scan or clinically by a consultant)
- Safe to be discharged home over the 24-hour period (e.g. adequate equipment, cognitive or physical support, and continence needs managed in-between care calls)
- Barnet resident (no need to be registered with a Barnet GP)
- Patient cognitively able to engage with rehabilitation program and has rehabilitation goals
- Able to transfer/mobilise with one person assisting
- Accept assistance of two patients but only if there is a family member or a carer at home to assist for the duration of the intervention
- Patient is medically stable and able to manage nutritional requirements safely at home
- New strokes within 6 weeks from stroke onset - for full ESD input for up to 6 weeks of therapy, any older stroke first time discharging from acute will only be seen for one-off assessment and will be handed over to ICS (clcht.plannedcarebarnet@nhs.net)
Exclusions
- Non-Barnet resident, medically unstable or unsafe of 24h, TIA, re-referral with chronic stroke symptoms (please refer to ICS, clcht.plannedcarebarnet@nhs.net)
