Camden Learning Disabilities Service provides health and social care to adults with a learning disability and their carers.

The service is multidisciplinary and includes social work, occupational therapy, community nursing, psychiatry, psychology, physiotherapy, specialist support and speech and language therapy.

Eligibility Criteria

Inclusions

Adults with a learning disability who:

  • live in Camden or
  • have a Camden GP
  • and the people that support them.

How to Refer

EMIS form

Referral methods: Email

GP referral by completing Camden Learning Disabilities Service (CLDS) Referral Form and sending by email. Please refer to the CLDS Referrals (see Downloads), which includes referral criteria and a consent form.
