Learning Disabilities Service

Camden

Camden Learning Disabilities Service provides health and social care to adults with a learning disability and their carers.

The service is multidisciplinary and includes social work, occupational therapy, community nursing, psychiatry, psychology, physiotherapy, specialist support and speech and language therapy.


Eligibility Criteria

Inclusions

Adults with a learning disability who:

  • live in Camden or
  • have a Camden GP
  • and the people that support them.

Complete the Camden Learning Disabilities Service (CLDS) Referral Form and email to clds@camden.gov.uk

If the person being referred has urgent social care needs, please identify them in the referral and contact the CLDS duty care manager by calling 020 7974 3761
