Community-based Service Transition Update:

The method of referral for community services have now been revised to either eRS or email:

Referral route to Barnet community services

Referral route to Camden community services

Referral route to Enfield community services

Specific service pages will also reflect any changes to referral route, referral form title and its location within EMIS Global folder.

Please follow the ‘Community first’ ethos with referrals, where available, and meet suitable service inclusion and exclusion criteria. This means that where there is an appropriate community service available, it should be highly considered.

Please note: where there is a community-based service in place, the minimum standard of five patient eRS choices does not apply.
