Routine imaging requests

  • Complete the Ultrasound Imaging GP Direct Access Request – UCLH form.

GP direct-access urgent suspected cancer imaging requests

  • Complete the Urgent Direct Access Imaging Request Form (adult & paed) Suspected Cancer – NCL form.

Email forms to ​​​ucl-tr.UCHRadiology@nhs.net

If imaging supports referral for urgent suspected cancer (USC), a separate USC referral form will need to be completed (see relevant USC service page for details, search 'urgent suspected cancer').
