Complete the Ultrasound Imaging GP Direct Access Request – UCLH form.

GP direct-access urgent suspected cancer imaging requests

Complete the Urgent Direct Access Imaging Request Form (adult & paed) Suspected Cancer – NCL form.

If imaging supports referral for urgent suspected cancer (USC), a separate USC referral form will need to be completed (see relevant USC service page for details, search 'urgent suspected cancer').

GP Gynaecology Ultrasound Service for <18 years

UCLH offers an gynaecology ultrasound-only service for patients under the age of 18. Contact the Gynaecology Diagnostic Unit at Lower Ground Floor, EGA Wing via uclh.admin.generalgynaecology@nhs.net

Please note: any patients over the age of 18 should be referred to Wood Green Community Diagnostic Centre for a pelvic ultrasound scan: woodgreencdc.whitthealth@nhs.net
