Heart Failure Community Service: CNWL

Camden

The Camden Heart Failure Service is a nurse-led service made up of specialist nurses who review patients with a confirmed diagnosis of heart failure.

Services include:

  • clinics at three different locations in the borough
  • telephone reviews or home visits for medication optimisation
  • fluid management
  • education to help patients self-manage their condition
  • referral to a cardiac rehabilitation programme specific to heart failure
  • opportunity to join the monthly Cardiac Support Group.

Weekly multidisciplinary team meetings take place with a consultant cardiologist and hospital heart failure cardiac nurse specialist to offer further support to the specialist nurses and to help reduce hospital admission.


Eligibility Criteria

Inclusions

  • aged 18+ and registered with Camden GP
  • diagnosis of heart failure supported by an echocardiogram and management plan, which should be less than a year old.

Exclusions

  • not suitable for people who require diagnosis or complex interventions that are normally only available from secondary care.
