DESMOND Type 2 Diabetes Education: CLCH
BarnetThe DESMOND programme offers a structured self-management education for patients with type 2 diabetes, focusing on lifestyle modification and behaviour change. The programme includes group-based education delivered virtually or in-person, covering key topics such as dietary assessment, understanding diabetes, and future planning. Available both in-person and digitally on the MyDESMOND platform.
Eligibility Criteria
Inclusions
- registered with a Barnet GP
- diagnosed with type 2 diabetes
- HbA1c > 48mmol/mol.
