Community Parkinson's Disease Service

Enfield

The Community Parkinson's Disease Service is delivered by a nurse specialist who aims to enhance the lives of people with Parkinson's disease through education, support and advice.

Patients are assessed before being recommended evidence-based interventions. The service works closely with hospital consultant neurologists, GPs and other healthcare professionals to offer medication management, resources, onward referrals and signposting. 


Eligibility Criteria

Inclusions

Patients who:

  • have a diagnosis of Parkinson's disease
  • live in Enfield or are registered with a GP Enfield

How to Refer

Referrals are accepted from all healthcare professionals.

Complete the Adult Services Single Point of Access Referral Form - Enfield Community Services and send it to the centralised Enfield Community Services: northmid.ecsadultsinglepointofaccess@nhs.net

Once the referral has been received, it will be screened by non-clinical administrators with no changes to clinical response times.

Self-referral

Patients who have been diagnosed with Parkinson's disease can self-refer by phone or email.
