Adult Community Rehabilitation Service
EnfieldProvided By
The Adult Community Rehabilitation Service works closely with colleagues from primary and secondary care, partners within the London Borough of Enfield, and voluntary sectors to provide patients with rehabilitation and education. It also signposts for social integration.
The service:
- provides appropriate walking aids to enhance balance and mobility, boost confidence and function, and reduce the risk of falls
- provides community rehabilitation to housebound adults in, or close to, their homes
- supports families and carers looking after patients at home and signposts them to relevant services
- supports older people who are experiencing falls to achieve a good quality of life, maximise independence and wellbeing, and reduce mortality and long-term disability
- works in a multi-disciplinary setting and onward referrals to appropriate services.
Eligibility Criteria
Inclusions
- Enfield resident
- are aged 18+
- registered with a GP
- patients who are housebound
- patients experiencing falls (not necessarily housebound)
How to Refer
Referrals are accepted from all healthcare professionals.

Complete the Adult Services Single Point of Access Referral Form - Enfield Community Services referral form and send to the centralised Enfield Community Services: rf-tr.ecsadultsinglepointofaccess@nhs.net
Once the referral has been received, it will be screened by non-clinical administrators with no changes to clinical response times.
