Adult Community Rehabilitation Service
EnfieldProvided By
Adult Community Rehabilitation Service enhances balance and mobility by providing walking aids, offers community rehabilitation to housebound adults, supports families and carers, aids older individuals experiencing falls to improve independence and well-being, and operates within a multi-disciplinary setting for onward referrals.
Eligibility Criteria
Inclusions
- Enfield resident
- are aged 18+
- registered with a GP
- patients who are housebound
- patients experiencing falls (not necessarily housebound)
How to Refer
Referrals are accepted from all healthcare professionals.

Referral methods: Email
Complete the Adult Services Single Point of Access Referral Form - Enfield Community Services referral form and send to the centralised Enfield Community Services: rf-tr.ecsadultsinglepointofaccess@nhs.net
Once the referral has been received, it will be screened by non-clinical administrators with no changes to clinical response times.
