Community Diabetes Service

Enfield

The Enfield Community Diabetes Service provides education, management support and advice to people living with diabetes, their carers, and other healthcare professionals.

The service offers:

  • education sessions for those newly diagnosed with type 2 diabetes (HbA1c ≥48 mmols/mol)
  • health and wellbeing coaching sessions - providing behaviour change advice and support
  • specialist dietetics input - providing education and detailed advice regarding dietary management of diabetes
  • management of tolerance issues with hyperglycaemic agents
  • management of those on the maximum dose of three hyperglycaemic agents with a HBA1c >65mmol/mol requiring insulin initiation/other injectables with an EGFR <40
  • hypoglycaemia management
  • management of steroids-induced diabetes
  • diabetes and PEG feed management
  • palliative care
  • diabetes care at home/residential and nursing home visits.

Eligibility Criteria

Inclusions

Patients

  • 18 years old and over
  • registered with an Enfield-based GP
  • with type 2 diabetes
  • with type 3c diabetes with negative GAD antibodies test
  • steroid-induced diabetes
  • with eGFR >40.

Structured education

  • within 12 months of diagnosis of type 2 diabetes.

Diabetes Nurse Specialist (DNS) Clinic

  • diabetes with Hba1c >65mmol/mol/8.0% despite being on maximum dose of 3 oral-tolerated OHA/GLP- 1 RA/insulin
  • hypoglycaemia
  • type 3c with negative GAD antibodies test.
  • steroid-induced diabetes
  • tolerance issues
  • pre-conception clinic
  • complications review.

Exclusions

Patients

  • under 18 years old
  • with type 1 diabetes
  • with type 2 diabetes and EGFR below 40 (needs to be referred via e-RS to secondary care)
  • with gestational diabetes
  • with mature onset diabetes of the young (MODY)
  • not on maximum dose of anti-hyperglycaemic oral agents with no tolerance issues.

DNS Clinic

  • type 3c with positive GAD antibodies test

Please note: Patients meeting the exclusion criteria need to be referred to secondary care.

Complete the Adult Services Single Point of Access Referral form - Enfield Community Services and email to the following centralised ECS address:

For any enquiries regarding complex referrals or awaiting referrals, contact: northmid.diabetes@nhs.net

Once the referral has been received, it will be screened by non-clinical administrators with no changes to clinical response times.


Please note: The service does not provide the Diabetes Prevention Programme (DPP). All referrals sent to the above address for the DPP will be rejected.
