Children & Young People's Continuing Care Team

The Whittington Health (WH) Children and Young People's Continuing Care Team aims to:

  • provide and deliver personalised care packages to children who meet the threshold for continuing care
  • deliver healthcare needs to children and young people in the home and community setting
  • support discharge from hospital of children with complex health needs who meet the threshold for continuing care
  • regularly review care and assessment health needs in partnership with the multi-professional services
  • complete the National Framework for Children and Young People's Continuing Care assessment according to local and national guidelines
  • provide supplies and equipment.

Eligibility Criteria

Inclusions

Children and young people who:

  • are aged 0-17 and have a GP in Camden, Haringey or Islington
  • have a complex health need that can't be met by existing universal or specialist services alone
  • meet the locally set threshold criteria in line with the National Framework for Children and Young People's Continuing Care document (see Downloads).

Exclusions

  • those with health needs that are met through existing universal or specialist health services.
