The Children and Young People's Continuing Care Team provides a service to children and young people who have very complex health needs, either due to congenital conditions, long-term, life-limiting, or life-threatening conditions, disability or the after-effects of serious illness or injury.
These needs maybe so complex, that they cannot be met by existing universal or specialist services which are routinely available from GP practices, hospitals or in the community.
The team provides support in the form of individualised care packages and is made up of qualified children's nurses.
Opening times: Monday-Friday, 9am-5pm
Eligibility Criteria
Inclusions
Children and young people who:
- are aged 0-18 and registered with a GP in the London Borough of Barnet
- have a very complex health need that cannot be met by existing universal or specialist services alone
- meet the criteria in line with the National Framework for Children and Young People’s Continuing Care document
