GPs connected to Whittington ICE system can refer directly

Otherwise, use the appropriate form 

• For routine requests, complete the Imaging Referral Form – Whittington form.
• For all GP Direct Access urgent suspected cancer imaging requests, complete the Urgent Direct Access Imaging Request Form (adult & paed) Suspected Cancer – NCL form.

Email form to whh-tr.ImagingAppointments-WhittHealth@nhs.net 
or patients can bring the request form to the Imaging Department, on Level 3. 

If imaging supports referral for USC, a separate USC referral form will need to be completed (see relevant USC service page for details).

Please note: patients should not contact the department to make an appointment.

When referring a patient who needs hospital transport or who has mobility difficulties, please clearly mark this in the referral. This will help ensure that Imaging sites at WH have the appropriate number of staff during the patient's appointment and while they wait for their transport home.
