Routine imaging requests

GP Direct Access urgent suspected cancer imaging requests

  • Complete the Urgent Direct Access Imaging Request Form (adult & paed) Suspected Cancer – NCL form and email to rf-tr.ultrasound@nhs.net

If imaging supports referral for urgent suspected cancer (USC), a separate USC referral form will need to be completed (see relevant USC service page for details, search 'urgent suspected cancer').
