The Bed-Based Intermediate Care Reablement Team consists of a multi-disciplinary team from key health and social care organisations (Whittington Health and Islington Social Services).

There are ten intermediate care beds available at St Anne's nursing home. The team will offer you time-limited support to regain your independence after a hospital admission and help you plan a safe departure home when you are 'functionally fit'. You will stay within the bed-based service at St Anne's for a limited time only up to a maximum of six weeks.

Eligibility Criteria

Inclusions

• Patients who have a GP within NCL and
• Who require a short-term stay or rehabilitation whose needs cannot be met at home because of:
  • needing assistance of two
  • dementia
  • bariatric
  • upper/lower limb NWB

Exclusions

If  a patient has the following, their needs cannot be met at St Anne's:

• NG or NJ tube
• CPAP
• Oxygen therapy
• Covid positive
• End-of-life care
• IV
• Suctioning
• PICC line
