Expert Patients Programme

The Expert Patients Programme (EPP) facilitates six weekly group sessions designed to empower individuals with long-term health conditions to self-manage effectively.
Participants can improve their self-management skills, potentially resulting in fewer GP appointments, ED visits, and better medication adherence.
Eligibility Criteria
Inclusions
- adults aged 18+
- registered with a GP or living in Haringey or Islington
- living with one or more long-term health conditions (e.g. asthma, arthritis, multiple sclerosis, depression, diabetes, COPD, heart disease, fibromyalgia, sickle cell, HIV, chronic fatigue, chronic pain)
- carer of someone living with one of more long-term health conditions
- English, Turkish, or Bengali-speaking.
How to Refer
Referrals can be made by emailing the Self-Management team directly.

Self-referral
Patients can self-refer by telephone, email, or online.
