Provided By

The Camden Case Management service supports effective care for people with long-term conditions and those with complex health and social care needs. 

 The service provides:

  • three dedicated complex care nurses and one dedicated CPN complex care nurse
  • support with discrete caseloads
  • clinical supervision of complex care nurses
  • avenue/fast-track to teams and services provided by CNWL Camden provider services

Eligibility Criteria

Inclusions

  • Any adult Camden resident with complex needs for whom an MDT discussion could support their care and wellbeing within the community.

Exclusions

Any person:

  • under the age of 18
  • not a resident in Camden.

Complete the Camden Integrated Adult Community Services - CNWL and send to camdenreferrals.cnwl@nhs.net

The complex care nurses can also be referred to by ticking the complex care nurse box on the referral form.
