The Camden Case Management service supports effective care for people with long-term conditions and those with complex health and social care needs. 

 The service provides:

• three dedicated complex care nurses and one dedicated CPN complex care nurse
• work across whole of Camden, including support with discrete caseloads
• clinical supervision of complex care nurses
• avenue/fast-track to teams and services provided by CNWL Camden Provider services.

The service is based in IPC DN Teams, East in Kentish Town Health Centre, North in Gospel Oak, South in Hunter Street, West in Belsize Priory.

Eligibility Criteria

Inclusions

• Any adult Camden resident with complex needs for whom an MDT discussion could support their care and wellbeing within the community.

Exclusions

Any person:

• under the age of 18
• not a resident in Camden.
