If you would like to seek advice from the Coroner, or if you would like to make a referral to the Coroner following a patient death due to accident, suicide, violence, neglect (by self or others) or industrial disease or death for which the cause is not known, complete the referral form Coroners Service Referral and send to: enquiries.northlondon@coronerservice.haringey.gov.uk
