Neuro Navigation and Motor Neurone Disease Co-ordinator Service

Neuro Navigation

The Neuro Navigation Service provides a coordination model for patients with acquired or traumatic injuries and complex progressive neurological conditions. It combines rehabilitation coordination with patient and care advocacy to streamline access to neuro rehabilitation. The service includes assessment, guidance, signposting, and referral management to ensure appropriate rehabilitation.

Motor Neurone Disease Co-ordinator

The Motor Neurone Disease (MND) Co-ordinator works as part of a multidisciplinary team to provide coordinated, patient-centred care for people living with MND. The service offers holistic assessment, information, education and support, helping patients and their families navigate services and access appropriate care across health, social care, palliative care and voluntary sector organisations.


Eligibility Criteria

Inclusions

  • Aged 18+ 
  • Registered with an Enfield GP or lives within Enfield
  • Have a diagnosed neurological condition acquired, traumatic or progressive 
  • Have a confirmed diagnosis of Motor Neuron Disease (Motor Neurone Disease Care Coordination only).

Referral methods: Email

Complete the Adult Services Single Point of Access Referral Form - Enfield Community Services and send to the centralised Enfield Community Services: rf-tr.ecsadultsinglepointofaccess@nhs.net

Once the referral has been received, it will be added to Neuro Navigator wait list and clinically screened.
