Community Rehabilitation Beds (step-up) Pathway
NCL WideProvided By
The Community Rehabilitation Beds (step-up) Pathway is for people who need intensive support with rehabilitation, reablement or recovery (up to three weeks), but do not require admission to an acute.
What's provided:
- rehab beds
- multidisciplinary team
- goal-focused rehabilitation
- return home support.
Eligibility Criteria
Inclusions
- 18 years or over
- registered with an NCL GP
- experiencing functional decline and have a specific rehabilitation goal/s to achieve
- unable to remain safely at home and has rehabilitation or reablement potential
- clinically stable but do not require acute hospital care
- consent obtained or best-interest decision made
- able to participate in intensive rehabilitation programme.
Exclusions
Medically unstable or requires acute diagnostics
- primary mental health admission
- end-of-life care
- requires long-term placement or respite care or rehousing
- infection prevention and control needs cannot be met by the unit environment.
Note: Flexibility of criteria is considered under exceptional circumstances and agreed by discussion with screener.
How to Refer
Referrals are accepted from all healthcare professionals.
EMIS form
Complete the Discharge Passport NCL and send to cnwl.nclicehub@nhs.net
Provide a copy of the Access Rehabilitation in NCL leaflet to the patient or next of kin. (See Resources)
